Regarding Advance Health Care Directives, while at a local hospital, with a friend, we were told that they would only accept Advance Health Care Directives published by their own hospital. You may want to consult with local hospitals, their legal department staff, or your own attorney regarding Advance Health Care Directives. I did find a site that has Health Directives for every state. I am not advising you to use this CA Health Directive however‚ it addresses the information and issues, you may want to discuss with your attorney.

Notaries can not offer legal advice. Notaries provide ministerial services and their powers are very limited. Unlike judicial officials who are given broad discretionary authority by the law. If a signer has a question about a document- before calling notary services - they should consult thier attorney - or the document’s issuing or receiving agency- with any such questions. It is especially important to consult an attorney about wills, probate or conservatorship documents, rather than using it do it your self kits.

A Notary Public may also be present and assist with verifying safe deposit box contents‚ and attest to the authenticity of photocopies of specific documents (Attesting to copies made in the presence of the notary).

It is required that signors sign all documents in person‚ with Notary present; Show proper identification and understand the terms of notarization.
